Please print out the forms you need, fill them out, and bring them with you to your first appointment.  If you are unable to access a printer, you may come to the office 15-20 minutes early in order to complete copies provided in the waiting room.

Initial Paperwork for New Clients (or for clients returning to counseling after more than a one year break)

Release of Information Exchange (gives permission for your health care providers to share clinical information about you, your conditions, and your treatment).
